Vision Insurance Coverage

Taking care of our eyes is an important aspect of our overall health. An annual eye exam, for example, even if you don’t suffer from near- or far-sightedness, can spot early warning signs and symptoms of diseases such as glaucoma, which, if left untreated, can cause irreversible eye damage and is a leading cause of blindness.

The Convention’s new insurance agency, CSBC Insurance Services, LLC (operated through the HR & church compliance ministry), has negotiated a benefit for all church employees and volunteers, full- or part-time, and their dependents through EyeMed Vision Services. This benefit-rich plan offers deep discounts on vision exams, prescription eyeglasses and contact lenses, and a large network of independent optometrists as well as LensCrafters, Pearle Vision, Target Optical, Sears Optical, and JC Penney Optical. There also are discounts for “cosmetic” procedures such as lasik vision correction.

The plan includes eye exams with an in-network copay of just $10, as well as a zero copay and $180 allowance for frames (and 20 percent discount on any amount over $180); zero copay for standard single, bifocal, trifocal or lenticular lenses; and $65 copays for standard progressive lenses.

Tier 1 through Tier 3 premium progressive lenses also are available with copays from $85 to $110, and Tier 4 premium progressive lenses have a $65 copay, and include a $120 allowance and a 20 percent discount on any amount over $120. Traditional and disposable contact lenses are included with zero copays and $180 annual allowances. Reimbursements are available for out-of-network purchases.

Unlike many vision plans that typically offer an annual eye exam but new frames and lenses only once every 24 months, the CSBC plan includes both an eye exam and new frames and lenses as often as every 12 months!

As a promotional benefit, EyeMed is offering members a zero copay and zero out-of-pocket cost for any available eyeglass frames at all Target Optical and most Sears Optical locations. In addition, through Sunglass Hut, EyeMed members can receive $20 off non-prescription sunglasses up to $199.99 and $50 off sunglasses priced at $200 or more.

There are no age restrictions. Premiums are just $11.48 per month for a single person, $21.81 for two persons or $32.03 for three or more.
